Cannabinoid Hyperemesis Syndrome Presumed Secondary to CBD Use: A Case Report

Please use this link to access this publication. Abstract Introduction Cannabinoid hyperemesis syndrome is characterized by recurrent episodes of severe nausea and vomiting, often associated with prolonged and excessive cannabis use. With the recent legalization and rising consumption of cannabidiol (CBD) in Europe and the United States, concerns have emerged about its potential role in triggering similar symptoms. Case Report A 32-year-old male with a history of cannabis, tobacco and alcohol use disorder experienced multiple cyclic vomiting episodes after switching from cannabis to CBD. Initially, the patient presented with abdominal pain and vomiting after ceasing cannabis use, with symptoms alleviated by hot showers. Three...

Practical approach to the safe use of cannabidiol in patients with refractory epilepsy: a mini review

Abstract Drug-resistant epilepsy (DRE), characterized by seizures that are unresponsive to the use of two or more conventional anti-seizure medication (ASMs), poses a significant therapeutic challenge. In recent years, cannabidiol (CBD), a non-psychoactive compound derived from Cannabis sativa, has emerged as a promising adjunctive therapy for specific, severe epilepsy syndromes. The approval of a highly purified 99% CBD oral solution (Epidiolex®) has facilitated its integration into clinical practice. However, its effective and safe use requires a nuanced understanding of several practical considerations. This narrative review synthesizes current evidence on the practical aspects of prescribing and administering highly purified CBD oral solution in patients with drug-resistant...

Stereoisomers of cannabidiols and their pharmacological activities – A potentially novel direction for cannabinoids

Abstract Cannabidiol (CBD), a bicyclic non-psychoactive cannabinoid biosynthesized by Cannabis spp. of plants, has attracted significant interest in the past decade due to its therapeutic properties. In 2018, the US FDA approved Epidiolex®, a CBD-based drug for the treatment of two rare epileptic seizure disorders. CBD possesses two chiral centers at C3 and C4 on its terpenoid moiety and exhibits cis–trans stereoisomerism along the C3–C4 bond axis. (−)-trans-(3R,4R)-CBD, the natural CBD, is biosynthesized by the cannabis plant, while the unnatural (+)-trans-(3S,4S)-CBD is obtained via chemical synthesis. Both trans isomers exhibit broad in vitro and in vivo biological activities; typically, the unnatural stereoisomer (+)-trans-CBD and its derivatives exhibited more potent activities in comparison to the corresponding (−)-trans isomers. On the other hand, cis-CBD isomers...

Cannabidiol and microRNAs: shared cellular targets and new insights for developing anti-seizure modalities

Please use this link to access this publication. Abstract Cannabidiol (CBD) and 9-tetrahydrocannabinol (THC) are the two main components of cannabis that provide its therapeutic benefits. CBD has been extensively studied for its role in reducing seizures, among its many other uses. While the exact mechanisms by which CBD works to relieve seizures have not yet been fully determined, it is evident that CBD effectively diminishes seizure activity and is now being used as an approved treatment for severe forms of non-responsive epilepsy. As essential components of several biological processes, microRNAs (miRNAs) are crucial for achieving optimal cellular functioning. During the past few years,...

A cautionary tale of paradox and false positives in cannabidiol research

Please use this link to access this publication. ABSTRACT Introduction Decades of research on cannabidiol (CBD) have identified thousands of purported cellular effects, and many of these have been proposed to correlate with a vast therapeutic potential. Yet despite the large volume of findings fueling broad optimism in this regard, few have translated into any demonstrable clinical benefit or even notable side effects. Therein resides the great paradox of CBD: a drug that appears to affect almost everything in vitro does not clearly do much of anything in a clinical setting. Areas covered Comparative critical evaluation of literature searched in PubMed and Google Scholar discovers multiple...

Safety, Efficacy and Doxorubicin Pharmacokinetics During Cannabidiol/Cannabidiolic Acid Rich Hemp Oil Use in Dogs With Lymphoma Undergoing CHOP Chemotherapy

Abstract Background The safety of oral cannabidiol (CBD) and cannabidiolic acid (CBDA)-rich hemp oil supplementation in dogs with cancer receiving chemotherapy has not been investigated. Objectives To evaluate the safety, tolerability, and influence on doxorubicin exposure over time of oral CBD/CBDA-rich hemp oil supplementation in dogs diagnosed with high grade lymphoma undergoing CHOP chemotherapy. Animals Client-owned dogs diagnosed with lymphoma. Methods Dogs were enrolled in this prospective, double-blinded, randomized, placebo-controlled clinical trial to receive either CBD/CBDA-rich hemp oil capsules or placebo during one cycle of CHOP chemotherapy. Primary outcomes evaluated included adverse events during chemotherapy, quality of life scores and doxorubicin area under the...

A high-fat meal significantly impacts the bioavailability and biphasic absorption of cannabidiol (CBD) from a CBD-rich extract in men and women

Abstract Cannabidiol (CBD), a specialized metabolite (phytocannabinoid) abundant in Cannabis sativa, is attracting increasing attention for its alleged health-promoting properties. The present study aimed to investigate the pharmacokinetics of CBD and its primary metabolite, 7‐hydroxy‐cannabidiol (7-OH-CBD), following a single oral dose of a CBD-rich Cannabis sativa extract, equivalent to 70 mg CBD, in healthy male (n=5) and female (n=6) participants. Using a randomized crossover design, the study evaluated the impact of a standardized high-fat meal compared to fasting on the oral bioavailability of CBD. Consumption of a high-fat meal significantly increases the bioavailability of CBD. The geometric mean ratio (GMR) of CBD Cmax was 17.4 (90%...

When Public Will Meets Legislative Resistance: Five Lessons from Ohio’s Issue 2 Cannabis Legalization Controversy

Abstract In this paper, I explore the challenges of legalizing cannabis in Ohio, focusing on the passing of the Issue 2 Bill, legislative resistance, and public response. I propose five strategies for effective policy change: persistent advocacy post policy change success, establishing strong coalitions, empowering grassroots movements, promoting rigorous data-driven research, and launching public education campaigns. I offer a detailed analysis of the interaction between public opinion and legislative action in cannabis legalization and its implications for substance-related policy change.

The Pharmacokinetics and Pharmacodynamics of a Hemp-Derived “Full-Spectrum” Oral Cannabinoid Product with a 1:1 Ratio of Cannabidiol to Cannabidiolic Acid and Delta-9-Tetrahydrocannabinol to Delta-9-Tetrahydrocannabinolic Acid: A Double-Blind, Placebo-Controlled, Within-Subjects Human Laboratory Study

Abstract Aim: To examine the acute pharmacokinetics (PK) and pharmacodynamics (PD) of a patented oral cannabinoid product containing a botanical hemp-derived “full-spectrum” extract with an approximate 1:1 ratio of cannabidiol (CBD) to cannabidiolic acid (CBDA) and delta-9-tetrahydrocannabinol (THC) to delta-9-tetrahydrocannabinolic acid (THCA). Methods: Healthy adults (n = 15) ingested soft gels containing 0 (placebo), and approximately 1, 2, and 4 mg/kg of total cannabinoids (combination of CBD, CBDA, THC, THCA, and other minor cannabinoids) in an ascending-dose order in four experimental sessions separated by ≥1 week (the placebo condition occurred randomly within the dose sequence). Mean doses (mg) of primary cannabinoids in the active drug...

Cannabidiol in Foods and Food Supplements: Evaluation of Health Risks and Health Claims

Abstract Background: Cannabidiol (CBD) is a cannabinoid present in the hemp plant (Cannabis sativa L.). Non-medicinal CBD oils with typically 5–40% CBD are advertised for various alleged positive health effects. While such foodstuffs containing cannabinoids are covered by the Novel Food Regulation in the European Union (EU), none of these products have yet been authorized. Nevertheless, they continue to be available on the European market. Methods: The Permanent Senate Commission on Food Safety (SKLM) of the German Research Foundation (DFG) reviewed the currently available data on adverse and potential beneficial effects of CBD in the dose range relevant for foods. Results: Increased liver enzyme activities...
